Well, it's like when you're really bad at keeping track of your shit around the house and then you go out to buy more new stuff, eventually you'll run out of space :-)

Causes? programmers, people - just bad ones or good ones in a rush or not seeing the implications of a particular code change. In programmer's defense, everyone has a bad day every now and then, and however brilliant you might be you will write at times code that you'll slap yourself for thinking "what the hell was i thinking back then..." :-)
