Just read/green continuously or red/green with two restarts.

Blank screen but sound could be resolution support issue or regional setting. If you're trying to play a game like PAL Timesplitters 2 and you've region swapped your Xbox to NTSC so you can play it in 480p you get that happening. The game only supports PAL and there are other both PAL and NTSC games...

Yes, that's an almost generic XBMC error when the existing installation is corrupted in some way and can. of course, also appear when using an incompatible skin version. Always better to download a full new version to another folder and test that works rather than to try and update a running version...

Oh its: OpenXDK (since corrected in the earlier post) the Open Xbox Development Kit not OpenXDH. Dumb of me not to realise that and might be why I didn't get any hits when Googling.

No, HeXEn or any other burned disc won't run on a a retail state HDD even if locked. You can not just put another new HDD in a softmodded Xbox and expect it to work. AFAIK there is nothing you can do about that MS dash behaviour at least not with a softmod. I'm not sure about a TSOP though as you're replacing the MS Retail dash BIOS with a custom BIOS and it may be possible to edit the autoplay function on/off. It may be possible with a chip too. It should depend only on what you've called the path in XBMC to each Games folder wherever it is. I'm guessing at some point you created two different sources so they show up as Games (the original source) and Games(1) when you created the second path as a different source. It may be possible to save some stuff from F but its safest just to reformat both the extended partitions correctly and start from scratch IMHO.
