Well, it's like when you're really bad at keeping track of your shit around the house and then you go out to buy more new stuff, eventually you'll run out of space 
Causes? programmers, people - just bad ones or good ones in a rush or not seeing the implications of a particular code change. In programmer's defense, everyone has a bad day every now and then, and however brilliant you might be you will write at times code that you'll slap yourself for thinking "what the hell was i thinking back then..."
